All registrations must be completed by May 1, 2010

A mandatory check-in is required at least one hour before your first game.

The following items are required for team check-in:
1. A team roster.

2. Player Passes and Coach Passes.
Note: Only players and coaches with passes will be allowed to participate, NO EXCEPTIONS.

3. You may bring no more than 5 guest players.
All guest players must present a player card along with a
Specialty Player Action form signed by the guest player's coach

4. Medical release forms for all players.
Only players with medical release forms will be allowed to participate.
(included on the UYSA participant registration form)

5. Out of state teams need to have travel papers.
